Substandard and falsified medicines
Medication safety · Version 3

Substandard or falsified medicines may fail to treat illness or cause harm. Packaging, appearance, a QR code or a database listing cannot by themselves authenticate the physical product in your hand. Obtain medicines through appropriate regulated channels and discuss concerns with a pharmacist or healthcare professional.
NAFDAC's Greenbook provides a form headed “Report Substandard Or Falsified Products”. It asks for product details, the suspected issue, purchase source and supporting evidence. Keep relevant packaging and purchase records where safe. A suspected adverse reaction is a separate safety concern: ask a healthcare professional to help report it through NAFDAC's current medicine-safety channel. A reporting form is not emergency care. Severe swelling, breathing difficulty or collapse requires immediate medical attention.
